Attendee Conduct
I understand that if my conduct is deemed to be rude, uncooperative, unprofessional, or if I am found to be intoxicated or in possession of alcohol or any illegal substance, I may be asked to leave, and my registration for the Workshop will not be reimbursed under any circumstances.
Liability Waiver
I understand that, while Tom Jackson Productions has taken every possible measure to ensure my safety at the Workshop, I am legally responsible for my own safety, health, behavior, and possessions.
Damage to Property
I hereby bear all responsibility for damage to any and all property that may result from my negligent or intentional act or omission while attending and participating in the Workshop. Also, under no circumstances will Tom Jackson Productions or the venue be held liable for any loss or damage of my personal belongings due to my participation in the Workshop.
Medical Treatment and Liability
Should I require emergency medical treatment as a result of accident or illness arising during my attendance and participation at the Workshop, I consent to such treatment. I acknowledge and I agree to be financially responsible for any medical or legal bills that may be incurred as a result of emergency medical treatment. I will notify Tom Jackson Productions verbally and in writing if I am at any time injured during the Workshop, or if I have medical conditions about which emergency medical personnel should be informed; however, I understand that Tom Jackson Productions is not legally obligated to act on that information in any way or to provide any medical service whatsoever to me.
Performance Liability Waiver
I hereby accept all risk to my health, property and of my injury or death that may result from participating in any musical performance during the Workshop and I hereby release Tom Jackson Productions, the venue, and their officers, employees, interns, contractors, sponsors and representatives from any and all liability to me, my personal representatives, estate, heirs, next of kin and assigns for any and all claims and causes of action for loss of or damage to my property and for any and all illness or injury to my person, including my death, that may result from or occur during my musical performance at the Workshop, whether caused by negligence of Tom Jackson Productions, the venue, owners, or representatives, or otherwise.  I also agree that I am personally liable for any damage caused to the venue from my set up, ingress, egress, and tear down for any performance purposes during the Workshop.
Video/Audio Authorization
I hereby authorize Tom Jackson Productions and their affiliates and assigns, unrestricted and unlimited license to capture, distribute and display throughout the world, for any use including but not limited to give away, sale, or website use, any and all audio, photographic, and video images of the undersigned recorded on video tape, audio tape, or any other tangible format, or any recording of my original music at any and all times during any Tom Jackson Productions’ Workshop or Seminar.  Tom Jackson Productions also has the right to make modifications to the audio, photographic, and video images or build upon them as necessary or appropriate for their use.
